Artist Profile Questionnaire

Share the story behind your work. A few sentences per question are welcome; add more detail wherever you have a story to tell. Our editors use your responses to write a profile. You may skip questions that do not apply. Answer boxes expand as you type, so feel free to give detailed responses.

Publication permission

I confirm that the information I have provided is accurate to the best of my knowledge and that I own or have the necessary permission to authorize use of the submitted images. I give Art Show Magazine nonexclusive permission to create and publish an artist profile based on my responses, quote from those responses, and use the submitted images with the feature and in promotion of that feature on the magazine’s website, social media, and email newsletter. My responses may be edited for length and clarity without changing their meaning. Copyright remains with the respective rights holders.
